Model Release Form

I hereby grant permission to Prime Edge Media and the business in the form below, the rights to my image, in video or still, and the likeness of the sound of my voice without any payment or consideration. I understand that my image may be edited, copied, exhibited, published, or distributed, and waive the right to inspect or approve the finished product wherein my likeness appears. Additionally, I waive any right to royalties or any other compensation arising to the use of my image or recording. I also understand that this material may be used in diverse settings, within an unrestricted geographic area.

By signing this release, I understand that this permission signifies that photographic or video of me may be electronically displayed via the Internet.

I will be consulted if any of my images or recordings are planned to be used in any other manner than listed above.

There is no time limit to the validity of this release, nor any geographic limitation to where these materials may be distributed.
